–  The acquisition was made possible by a $47.5 million investment from S2G Investments (“S2G”), a multi-stage investment firm focused on scaling solutions across food and agriculture, energy, and oceans, in strategic partnership with TT Capital Partners, which first invested in Pyx Health in 2023. The acquisition combines the companies’ deep domain knowledge in healthcare and food systems to drive innovation at the intersection of two historically siloed sectors.

Pyx Health, a leader in engagement and care navigation, has partnered with FarmboxRx to deliver a more personalized and scalable approach to supporting vulnerable populations. The collaboration integrates compassionate, mentor-driven care with condition-specific nutritional support, addressing both non-medical drivers of health (NMDOH) and clinical engagement needs. This joint effort aims to improve member trust, close care gaps, and drive measurable improvements in cost, quality, and access.

Combining Navigation with Nutrition for Impact

• Pyx Health provides care navigation via trained mentors, using data-driven care tracks to address social and physical needs—especially loneliness and other NMDOH
• FarmboxRx delivers customized food boxes aligned with members’ health conditions and goals, enhancing health outcomes through improved nutrition

Together, the platforms address food insecurity—identified as the most common social barrier among Pyx Health members—while supporting health plans in incentivizing engagement behaviors like preventive visits and health assessments.
